Performs routine (journey-level) case management work in a fast-paced environment to meet the needs of individuals with and without intellectual and developmental disabilities; Work involves developing and maintaining long-term contact with clients, client families, and employment agencies/work sites for employment, medical, behavioral, social, educational, and related service needs; May train others; Work under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Essential Functions: Participates in job-coach training and skill development process for individuals enrolled in the program(s); Works with program coordinator to gather baseline data and assess individual(s) skills; Reinforces employability skills and supports the individual by understanding social aspects of the workplace; Makes appropriate modifications necessary to the successful completion of the job; Attends job orientation with the individual and clarifies information; Teaches essential tasks, duties and core skills of the job to the individual; Provides support depending on the needs of each individual; Builds natural supports to reduce the time spent with each individual as core skills are developed, with the understanding that independence in the work place is the ultimate goal; Demonstrates and teaches best safe practices of the work place/work environment; Models appropriate workplace behavior at all times; Regularly meets with the program coordinator and program manager to discuss issues and solve problems involving the individuals; Assesses individual progress and give feedback to individuals within the program and program coordinator; Completes assessments, reports and other documentation in a timely manner; Reports to program coordinator and manager; Communicates effectively with families, employers and program coordinator; Prepares/creates supporting materials to support lesson plans in approved curriculum for any and all grant programs; Conducts training presentations and instruction for clients to promote and explain employment programs, services, or approved curriculum; Devise solutions to problems as they arise.
Prefer knowledge of Intellectual and Developmental Disabilities, Autism, Related Conditions Services and funding sources; Knowledge of community resources, of case management delivery systems, principles, objectives, standards, methods, and of program policies and procedures; Skill in overseeing, developing, implementing, and monitoring service and treatment plans; Ability to assess clients’ needs, to oversee and coordinate clients’ services, to monitor program effectiveness, to communicate effectively, and to assign and/or supervise the work of others; To train others; Knowledge of rehabilitation programs for individuals with developmental disabilities; Skills in the use of a computer and applicable software; Ability to prepare and maintain records; Extensive travel within Bexar County required; Must have reliable transportation and Valid Texas driver's license; Ability to lift 30 – 50 pounds is required; Adequate health and physical mobility to get up and down off the floors as required First/CPR provision and application of SAMA techniques; Perform other job related duties as assigned.
Desired Degree/Job Experience: Bachelor’s degree in behavioral or Social Service field required with coursework in Social, Behavioral, Human Services or a related field; One (1) year or more of full-time work experience in fields of Education, Vocational Rehabilitation, Workforce Development programs, Human Services programs, or organizations serving individuals with intellectual or developmental disabilities required; Or any combination of education and experience which provides the necessary knowledge, skills, and abilities to sufficiently perform job duties; Must obtain and maintain University of North Texas Credentialing in: Job Skills Training, Job Placement, Supported Employment, Work Readiness and Autism Endorsement; A varied and successful work history; Experience working with individuals with disabilities; Time management skills and ability to manage multiple tasks is necessary
Valid Texas driver's license and reliable transportation; some local travel will be required. Prefer bilingual fluency in both English and Spanish.
