Screening and Eligibility
A psychological evaluation, called a Determination of an Intellectual Disability (DID), is conducted in accordance with state law and HHSC rules to determine if a person has an intellectual disability, developmental disability, or a related condition, or is a member of the HHSC intellectual and developmental disability priority population.

Individuals who are assessed with intellectual and developmental disabilities must have any of the following criteria to receive services:

  • IQ of 69 or below established by 18th Birthday (Intellectual disability)
  • Autism Spectrum Disorder (symptoms before age 10; Diagnosed before 22nd birthday)
  • Individuals with Related Conditions may qualify for limited services based on the disability, IQ level and adaptive behavioral level (Diagnosed before 22nd birthday)
  • Early Childhood Intervention (0-3 years old)

To begin the process, complete the application below and return by mail or hand deliver to Alamo Area Council of Governments, IDD Services, Attn: Intake & Eligibility Unit., 2700 NE Loop 410, Suite 101, San Antonio, TX 78217.

When we receive your application, the Service Coordination Specialist assigned to you will contact you to schedule an appointment and answer any questions you have.
